Mapping Global Trends in Community Health Service Design: Hotspot Evolution and Localization Strategies via Multidimensional Scaling-social Network Analysis

Abstract

The work aims to investigate research hotspots and evolution pathways in international community health service design, to provide theoretical guidance for improving service design interventions in China's community health quality enhancement. A systematic search was conducted across seven databases (Web of Science Core Collection, PubMed, Embase, Scopus, ScienceDirect, ProQuest, CINAHL) for literature on community health and service design. BICOMB software was used to extract keywords and construct document-term and co-occurrence matrices. SPSS was used to facilitate cluster analysis and multidimensional scaling analysis, while UCINET was employed to conduct social network analysis. From 1,379 identified studies, cluster analysis revealed four focal themes, namely community health service integration, inclusive service design and quality improvement, health equity, and multi-stakeholder digital health services. Multidimensional scaling analysis indicated distinct research statuses and developmental stages across these themes. Social network analysis demonstrated a hierarchical structure centered on keywords such as primary healthcare, service delivery, and patient engagement. In conclusion, future research should prioritize integration and innovation of community health services with digital empowerment and human-oriented approaches, the deepening of inclusive community service design through localized practices and method development, and the construction of a community health digital ecosystem oriented towards fairness and collaborative governance.
